Scheduling
This form is simply to initiate establishing care with LYHL Coaching & Physical Therapy (formerly Artisan Physical Therapy) and to request an appointment.
Your appointment will be confirmed with you once scheduled. If you prefer a phone call response, rather than an email response, please leave a phone number in the message field, but be aware that phone contact is more difficult to achieve due to scheduling challenges.
To maximize your visit time, you will receive a few simple forms to fill out prior to your arrival. When you arrive for your visit, wear flexible clothing to allow movement and examination of the area of interest and surrounding area as well.
Make sure to think about your movement goals - being as specific as possible to facilitate optimal communication.
Visit length & cost
in-person evaluations are 75 minutes and $265.
Follow-up visits can be 75 min ($247.50), 50 min ($165), or 25 min ($87.50) in length based on your needs and goals. Fees for remote visits are 10% less.
Cash, check, and credit card (Visa, Master Card, American Express, and Discover) are accepted.
To heighten the quality of the care I offer, I do not bill insurance nor provide superbills.
Payment due at time of visit.
HSA & FSA accounts can be utilized for physical therapy treatment payment.
